U bent van harte welkom om u in te schrijven bij onze praktijk als u woont in een wijk met de postcode 3014, 3015, 3021, 3022 of 3023. Dit kunt u op twee manieren doen:

1. Via deze link kunt u online uw inschrijfformulier invullen. Dan verwerken wij deze voor u zonder dat u langs hoeft te komen op de praktijk. 

2. U kunt een inschrijffformulier ophalen en invullen, vervolgens het ingevulde inschrijfformulier op de 2e etage aan de assistente overhandigen. Let op dat u wel een geldig legitimatie meeneemt, zonder kunnen wij uw inschrijving niet compleet maken.

Als u besluit zich bij ons in te schrijven, breng dan uw oude huisarts op de hoogte en vraag of hij uw dossier naar ons opstuurt.

You are welcome to register at our practice if you live in a neighborhood with the zip code 3014, 3015, 3021, 3022 or 3023.
You can register in two different ways:

1. Through this link, you can fill in the registration form online. We will process this for you, without you having to come by to our practice.

2. You can collect a registration form at our receptionist on the ground floor. You can then hand over the completed registration form to the assistant on the 2nd floor. Please note that you bring a valid ID, without it we cannot complete your registration.

If you decide to register with us.
please inform your old GP and ask him to send us your file.
